Requirements
1- Application form for duplicate card for loss,聽聽duly filled in our web portal.
2- Certificate of loss of the residence card聽, issued by the national police.
3- Copy of the passport face.
4- Two (2) front-sized photographs (2 脳 2),聽聽without jewelry or accessories and with bare ears, white background.
Notes:
1-聽This service must be requested exclusively by the interested foreigner or by proxy through duly legalized power of attorney (article 125 of regulation 631-11 for the application of law 285-04 on migration).
2-聽In the case of being defeated, you must renew it
Procedure to follow
1. Enter the DGM service portal.聽(聽https://personal.migracion.gob.do/聽)
1.1. Register in the system (if you are registered, enter your username and password)
1.2. Choose the “application list” option, from the list of available services, choose the one you requested, and complete the required information on the forms.
1.3. The system will notify you about the status of your request
2-聽The scanned documents must be in .jpg format for the application via web.
3-聽Present to the DGM with the original documentation plus a complete set of copies.
4-聽All documents in another language must be translated into Spanish, by the Dominican consulate in the applicant’s country of origin or by a judicial interpreter in the Dom.
5-聽All documents deposited in the DGM must be original and must be duly notarized, legalized or apostilled as appropriate.
