Requirements
1- Letter of application for the Consular Certification for loss of visa passport,聽聽addressed to the Director General of Migration indicating that this certification is requested because it is one of the requirements established by the Consular Missions in order to determine its entry or exit from the Dominican territory, indicating the period that you are interested in certifying as an entry or exit movement, depending on the requirements of each consulate.
You can authorize a third person to make the request, through a notarized power of attorney duly legalized and certified in the Attorney General’s Office, attached to the copies of the Identity Cards of the witnesses and weighted.
2- Copy of the Identity Card or any identity document of the applicant.
4- Certificate of loss of passport issued by the National Police.
Procedure to follow
1. Enter the DGM service portal.聽(聽https://personal.migracion.gob.do/聽)
1.1. Register in the system (if you are registered, enter your username and password)
1.2. Choose the “list of certifications” option, from the list of available services, choose the one you requested, and complete the required information on the forms.
2-聽The scanned documents must be in .jpg format for the application via web.
